💸 Salary Range: $80,000 – $150,000 (commensurate with experience)

📋 Conduct Comprehensive Assessments & Treatment Plans: Perform in-depth physical therapy evaluations and diagnostic tests to create personalized care plans in alignment with physician directives.

⏱️ Timely Evaluations: Complete initial patient evaluations within 48 hours of referral, ensuring accurate documentation and adherence to agency standards.

🤝 Collaborate with a Multidisciplinary Team: Work with physicians, case managers, and other healthcare professionals to communicate progress, care plans, and adjustments.

📚 Educate & Empower Patients: Provide essential education and empower patients and their families with knowledge on therapy needs, treatment options, and available resources.

📊 Lead Discharge Planning & Quality Initiatives: Contribute to discharge planning and quality improvement efforts, using tools like OASIS to measure patient outcomes and enhance care.

🛡️ Ensure Safety & Excellence: Maintain the highest standards of safety, infection control, and quality to provide a secure and positive environment for both patients and staff.

👩⚕️ Mentor & Support Home Care Team Members: Supervise and mentor aides and therapy assistants, ensuring high-quality service delivery in line with agency guidelines.
✅ What We’re Looking For:

🎓 Qualifications: Graduate of an accredited Physical Therapy program with strong clinical training.

📜 Licensing: Valid state license/registration as a Physical Therapist in Arizona.

⏳ Experience: Minimum of 2 years in healthcare, with preference for home health experience.

🩺 Certifications: Current CPR certification and a valid driver’s license with an insured vehicle.
